Request Medical Records
For a copy of your medical records:
- Please click the link below to submit a medical records request. Please DO NOT submit your request to the practice in person.
- Fees for medical records are mandated by the state of Florida and are as follows: $1 per page for the first 25 pages and $.25 cents for every page thereafter. You will receive an invoice for the exact page count for the records you have requested. Once paid, you will be able to download your records immediately.

- Records can be viewed
