Effective 9/1/2025, FMLA and Disability Forms for our patients will be processed by our practice.  Please bring the form(s) to our office;  pre-payment is required.  You will be asked for payment upon submitting the form at our office, as well as how you wish the completed form to be returned.  Normal turnaround time for forms completion is five (5) business days.   To check on the status of a submitted form, please email us at: forms@southflaortho.com
